What this policy covers
This policy covers benmati.com and its contact form. It does not cover your medical records. If you become a patient, your health information is protected under HIPAA, and you will receive a Notice of Privacy Practices at intake.
What I collect
- The contact form: your name, email address, what you are interested in, and anything you choose to add.
- Basic technical information: like most websites, the hosting service may keep standard logs, such as IP address and browser type, for security and to keep the site running.
Please do not include medical details in the contact form. It is not meant for health information.
How I use it
I use what you send only to write back to you and to schedule a call. I do not sell or rent your information, add you to a mailing list, or use it for advertising.
Who else handles it
The website host and the form service process this information for me so the site can work. They are not allowed to use it for their own purposes. This site is hosted by Cloudflare. Contact form messages are delivered to my email by Web3Forms.
Cookies and analytics
This site does not use advertising cookies or tracking.
How long I keep it
If we do not go on to work together, I delete contact form messages within 12 months. If we do, your information becomes part of your medical record and is kept as the law requires.
Your choices
You can ask to see, correct, or delete the information you sent through this site. Use the contact form and I will respond within 30 days.
Changes
If this policy changes, the new version will be posted here with a new date.
